What a care manager does
A care manager looks at your whole health: physical, mental, and social. They talk with you about your goals and help you:
- Get involved in meaningful activities that keep you healthy
- Obtain housing, legal assistance, food, and other essentials
- Find social and community activities to include in your life
- Move successfully from one type of care to another
- Keep everyone involved in your care aligned around your goals
Who's eligible
Medicaid recipients, including people with both Medicaid and Medicare, who meet one of these conditions:
- Two or more chronic health conditions (such as asthma, diabetes, heart disease, a mental health condition, or substance use disorder)
- A significant mental illness
- Living with HIV/AIDS
What it costs
Nothing. Medicaid pays for Care Management services for those who are eligible. There is no cost to enroll or participate. Services are voluntary; you'll be asked for consent during outreach.
