Health Home Care Management

A care manager in your corner — coordinating health care, housing, food, and the everyday needs that keep you well. For Cattaraugus County residents on Medicaid.

What a care manager does

Health Home services are about more than physical health — they’re about your mental health and social health too. Care Managers talk with you about your goals, needs, and what makes your life meaningful, then make sure those needs are met. They help you:

  • Get involved in meaningful activities that keep you healthy
  • Obtain housing, legal assistance, food, and other essentials
  • Find social and community activities to include in your life
  • Move successfully from one type of care to another
  • Keep everyone involved in your care aligned around your goals

Who's eligible

Medicaid recipients — including members of Managed Care Organizations and people with both Medicaid and Medicare — who meet one of these conditions:

  • Two or more chronic health conditions (such as asthma, diabetes, heart disease, a mental health condition, or substance use disorder)
  • A significant mental illness
  • Living with HIV/AIDS

What it costs

Nothing. Medicaid pays for Care Management services for those who are eligible — there is no cost to enroll or participate. Services are voluntary; you’ll be asked for consent during outreach.
